Cages with urethral rod?

Anyone have any experience with this kind of cages? My wife wants to have as much access as possible so that she can tease me, so I thought this would be the perfect cage. But I'm a little scared of infections and how it works when wearing it prologed periods.

I am afraid of urethra insertions. I have got this phobia from hospital. I think the cages have this bit as optional.

But I am also curious, anyone worn this sounding for long time? How long?
