Painful Panties for Males?

Any ideas for how to adapt the torture panties (tacks, sandpaper, etc) for someone with a penis? I worry that since the penis and scrotum don't lay as evenly, the pressure/friction wouldn't distribute evenly and it would go from "Painful but won't damage you" to "Will leave you bloody"
